Objective: To employ a first-order Markov chain to assess annual persistence and transitions in global health stratum, previously established by PCA and k-means clustering based on nine WHO health indicators. Design: A retrospective examination of country-year health profiles utilising nine WHO criteria from 2000 to 2024. We assessed transition probabilities among health burden strata and computed "time to healthiest stratum" metrics utilising survival analysis techniques to effectively manage uncertainty and right-censoring.
