Identification of S100A9 as a target for diagnosis and treatment of Crohn's Disease after Vedolizumab treatment failure

Recent advances in Whole-Transcriptome Sequencing technologies enable the complete sequencing of entire genomes and allow us to analyze a number of disease genome profiles. In our study, whole-transcriptome sequencing was performed based on the peripheral blood leukocytes of patients in the Non-responsders group (contain 5 CD patients who are nonresponse to vedolizumab treatment) and the Rseponders group (contain 3 CD patients who are response to vedolizumab treatment). Subsequently, differential messenger RNA (mRNA), circRNA and lncRNA expression analysis was performed of the Non-responsders and Responders groups, followed by functional interaction prediction analysis. This study may provide novel insights into the molecular basis of initiation and progression of IBD, and provide potential biomarkers for distinct prognostic and therapeutic implications. Overall design: To make the next treatment plan for unresponsive patients and select Vedolizumab for the right Crohn's patients to achieve precise treatment and better treatment persistence,this study used the Peripheral blood leukocyte samples which were from eight CD patients (five non-responders and three responders; age range: 18–52 years, mean age: 28.5 years) who accepted Vedolizumab treatment at the Second Affiliated Hospital of Zhengzhou University. Using a week's worth of observational scoring, the Crohn's Disease Activity Index (CDAI) is an essential indicator for calculating how active a patient's condition is. We defined responder as a patient with a CDAI <150 along with a Simple Endoscopic Score for Crohn's Disease (SES-CD) <5 assessed at least 14 weeks after the initial administration of vedolizumab; conversely, we rated it as a failure to respond if the patient did not meet the aforementioned criteria or suffered a dose increase that was not due to weight gain. All of the sections were graded according to their histological characteristics with the help of a pathologist.
