Neonatal jaundice associated with intrauterine infections (IUI) often reflects pathological liver involvement and systemic dysfunction. The aim of this study was to identify clinical and laboratory parameters that enable early differentiation between IUI-associated and physiological hyperbilirubinemia. A total of 72 neonates were examined: 42 with IUI and 30 with physiological jaundice. In neonates with IUI, total bilirubin on the 3rd day reached 248.6 ± 18.4 µmol/L, with the direct bilirubin fraction accounting for 26.8 ± 4.2% (p < 0.05). Early onset of jaundice, an increase in the direct bilirubin fraction, and elevated liver enzymes serve as reliable indicators of pathological progression and allow timely adjustment of neonatal management strategies.
