Our study explored the association between the admission level of serum cystatin C(CysC) and short-term functional outcome in patients with hypertensive intracerebral hemorrhage (HICH). Acutely ill patients with HICH (HICH group) were consecutively recruited after admission and were followed up for 3 months after admission. During the same period, patients with essential hypertension (EH group) were enrolled as controls. The HICH group was further divided into subgroups according to sex, age, hematoma volume, location of hematoma, admission National Institutes of Health Stroke Scale (NIHSS) scores, admission Glasgow Coma Score (GCS), and intraventricular hemorrhage or not, as well as a poor outcome subgroup and a good outcome subgroup at 3 months after admission by modified Rankin Scale (mRS) scores. Receiver operating characteristic curves were used to assess the predictive value of serum CysC for outcome in patients with HICH.Serum CysC levels were lower in the HICH group than in the EH group.Serum CysC levels were higher in patients with HICH with a hematoma volume ≤ 30 mL, NIHSS scores ≤ 10, and GCS 13–15 at admission (all <em>P</em> &lt; 0.05). In the HICH group, serum CysC levels were lower in the poor outcome subgroup and a lower serum CysC was associated with a poor short-term functional outcome in the HICH group, and 1.015 mg/L was the optimal cut-off value of serum CysC to predict outcomes at 3 months after HICH (<em>P</em> &lt; 0.001). The expression of serum CysC is lower in patients with HICH than in those with EH, and lower serum CysC is associated with poor short-term functional outcome in HICH.
