The National Lung Screening Trial (NLST) compared two methods for lung cancer detection: low-dose spiral computed tomography (CT, commonly referred to as spiral CT) and standard chest radiography. Spiral CT acquires multi-image scans of the entire chest using X-rays, while standard chest radiographs produce a single image of the entire chest where anatomical structures overlap with one another. Study findings showed that participants who underwent low-dose spiral CT scans had a 15% to 20% lower risk of dying from lung cancer compared to those who received standard chest radiography. This equated to approximately 3 fewer lung cancer deaths per 1,000 participants screened in the CT group relative to the chest radiography group over an approximate 7-year observation period (17.6 per 1,000 and 20.7 per 1,000, respectively). Across the three screening rounds, an average of 24.2% of low-dose spiral CT screenings returned positive results, while 6.9% of chest radiography screenings did so. For both trial arms, the majority of positive screening outcomes led to additional diagnostic tests.
