VPRS 18377 Admission Index Cards

This series consists of admission index cards of patients at Ararat (VA 2841). The index cards recorded details including patient initial (surname), name of institution, name, age, sex, marital status, occupation, religion, date of reception, previous admissions, reason for reception; section of Lunacy Act or other Mental Hygiene Acts applicable to committal, certifying medical practitioners, previous place of abode, names and addresses of relatives, names and addresses of persons signing request to receive, mental and physical condition, name of medical superintendent, date transferred and destination, date boarded out, date allowed on probation, date of discharge and reason, section 88 examination dates and date of death.Other information may include admission details to other institutions, birthplace, last school attended, education standard achieved, cause of death, name of staff member present at death, U.R number, pensioner number and a note that explains affairs are controlled by the Public Trustee.The early versions of the index cards (pre-1940) provided for single admissions and a reference to the warrant numbers was added retrospectively. Later versions of the index cards (post-1940) provided for details of multiple admissions.
