All odds ratios were compared with Sweden–born men in the corresponding time period. Model 1 includes: age at diagnosis in 5–year categories. Model 2 includes: variables in model 1+ education level. Model 3 includes:variables in model 2+ medical conditions. Model 4 includes: variables in model 3+ availability of a CCU. Bold numbers indicate statistically significant ORs.
