"PATHOPHYSIOLOGICAL SYNDROMES AND CRITICAL COMPLICATIONS OF COEXISTING DIABETIC NEPHROPATHY AND ACUTE HEMORRHAGE"

Diabetic nephropathy (DN), or diabetic kidney disease (DKD), remains the leading cause of end-stage renal disease (ESRD) globally, characterized by progressive glomerular and tubulointerstitial deterioration. When acute hemorrhage occurs on this compromised premorbid background, it triggers an abrupt, severe disruption of systemic and renal hemodynamics. This paper reviews the synergistic complications that arise when diabetic nephropathy and acute blood loss coexist. We analyze the failure of renal autoregulation, the acceleration of acute-on-chronic kidney injury (AKI-on-CKD), the exacerbation of the cardiorenal anemia syndrome (CRAS), and the profound bone marrow hematopoietic dysfunction driven by hypercatecholaminemia and hepcidin-mediated iron restriction. Furthermore, we outline clinical phenotypes (gastrointestinal, intracerebral, and retinal hemorrhages), evidence-based emergency resuscitation protocols, and critical medication stewardship guidelines to optimize survival outcomes in this high-risk patient population.
