It tests 8 arms × 100 scenarios = 800 decisions: ALLOW (no escalation required OR valid approval), HOLD (approval required but missing/invalid). Verdict: PASS (g_allow_no_esc=1.0000, g_allow_approved=1.0000, g_hold_no_approval_min=1.0000, g_margin=0.9500). Dual-guard concordance: 800/800 (100%). Kill-gate: 100/100 wrong-allows (falsifiable). Ensures high-risk API operations requiring human oversight cannot proceed without valid approval.
