Reprinted with permission.†The body-mass index (BMI) is the weight in kilograms divided by the square of the height in meters.§The glomerular filtration rate (GFR) was estimated by using the Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) equation [48].¶Chronic kidney disease was classified according to the Kidney Disease Outcomes Quality Initiative of the National Kidney Foundation. Stage 1 denotes a glomerular filtration rate (GFR) of 90 ml or more per minute per 1.73 m2 of body-surface area; stage 2, a GFR of 60 to 89 ml per minute; and stage 3, a GFR of 30 to 59 ml per minute.‡Patients' self-reported medical histories at randomization.
