Correction-Path Census: Dataset, Coding Protocol and Reproducibility Materials for Better Answers, Worse Decisions

This deposit contains the data, frozen coding protocol, deterministic analysis and Figure 21 for the 24-case documentary correction-path census reported in 'Better Answers, Worse Decisions'. Six institutional capacities are coded on a 0–2 scale: detection, independent challenge, diagnostic competence, stop or reversal authority, remedy or recovery, and institutional learning. External compulsion is coded separately. Cases are classified as timely correction, delayed or hybrid correction, or failed or severely delayed. Within the purposively bounded case universe, detection was directly documented in all 24 cases. Stop or reversal authority was absent at the critical point in all five failed or severely delayed cases. A minimally connected correction path appeared in all eight timely cases, ten of eleven delayed or hybrid cases, and none of the five failed cases. The evidence is descriptive and author-coded. It does not estimate prevalence, establish causation, claim representative sampling, or report independent inter-rater reliability. This release was deposited after the primary author coding and is not represented as a preregistration of that coding. It prospectively fixes the independent recoding materials from the deposit date.
