These are the forms by which patients or their guardians gave their consent for the performance of certain medical procedures upon themselves. The procedures covered are Operative Treatment and Administration of Anaesthetic, and E.C.T. (Electro-Convulsive Therapy, or "shock treatment"). The same form provides for consent to either procedure, and also allows for the consent of parents to treatment of a minor, or for the consent of relations to the treatment of a person incapable of giving his or her own consent. This appears to be merely a section of a much longer series of original records, and, like certain other series transferred from this agency, may have been intended merely as a sample of records, to be retained for historical purposes. A tag accompanies these forms, bearing the word "Archive," which would seem to bear out this conjecture.
