These forms contain the referral for a patient's transportation and admission to hospital, and the nurse's in-flight records on the patient. The slips are arranged in a running annual sequence and include a separate numerical sequence for helicopter bookings. The running number sequence is also chronological(13/11658-88). 31 boxes.
