Report a website issue

This is a Survey123 form that allows users to quickly report an issue with one of Planning & Development Department's web mapping applications. When users click on the “Report Issues” link on any of our web application, they are able to fill out this Survey123 form that contains contact information for the user, name of application with the issue, a description of the problem. If necessary, they can also upload a screenshot of the problem. The information from this report populates the table of a feature layer. The GIS Team can review this information and then take necessary action to resolve the issue and to contact the user with results. Any questions about this layer can be directed to [e-mail].
