TGA COVID-19 Vaccine Safety Monitoring Plan — Implementation and Governance Evidence Dataset (2021–2026)

The dataset underlies an independent documentary audit assessing implementation of the TGA COVID-19 Vaccine Safety Monitoring Plan (February 2021) across 68.4 million COVID-19 vaccine doses administered under provisional approval. The audit applies ISO 19011:2018 methodology and Australian national auditing principles, including the principles of transparency, verifiability, permanence, and accountability.All summary statistics, status distributions, cross-tabulations, and severity aggregations reported in the audit are directly reproducible from the underlying evidence matrix using standard analytical tools. The audit's open review architecture is documented at Open Review Architecture, including the methodology, evidence base, version history, and analytical decisions.
