VPRS 18933 Patient Registers and Case Records

This series comprises Admissions Registers, Roll Books, Death Registers, and Case Books of Mount Royal Hospital (VA 1280). The latest Death Register also includes records from the Melbourne Extended Care and Rehabilitation Service (VA 5265), the organisation which resulted from the amalgamation of Mount Royal Hospital and the Greenvale Centre (VA 5267). The Roll Books are listings of inmates which include information such as name, where admitted (i.e. LaTrobe Street, St Kilda Road or Royal Park), birthplace, age, condition (marital status), religion, occupation, when arrived in colonies, name of ship, residence last five years, ward number, discharge date, whether died, if a pensioner, and cause of death. The earliest Roll Books (1888 – 1924 and 1899 – 1924) overlap, however different details are recorded within them.  The Admissions Registers are listings of inmates that at various times record information including admission date, name, previous address age, birthplace, religion, occupation, year arrived in Australia, name of ship, whether outpatient or convalescence at another hospital, residence, shelter for night, meals, date of discharge or death, if old-age pensioner, reason for discharge, and additional remarks.  Death registers record information including registration number, date of death, name of patient/inmate, age, religion, birthplace, occupation, ward number, date admitted, name of undertaker, cause of death, doctor's name, hour of death (in later periods), sex, if ring on finger, signature of attendant receiving remains, date body collected, whether burial or cremation, and signature of mortuary attendant delivering remains. Case books record information including name, age, occupation, dates of admission, treatment and discharge or death, as well as a record of patient symptoms and administered medical treatment.  The Dismissal Register records information including: name, date, description of the infraction, behaviour or incident that lead to the dismissal of the patient/inmate, and the name of the person writing the submission, which is often the Medical Superintendent.
