Number of near miss medical errors reported to JQ Japan 2023, by incident type

In 2023, approximately 10.48 thousand cases of medical near misses related to medicines were reported by medical facilities to the Japan Council for Quality Health Care (JQ). Near misses refer to mistakes that were almost made or actually made but got found before they resulted in harm to the patients. That year, a total of about 28.88 thousand medical near misses were reported to JQ. Designated by the health ministry in the country, JQ conducts various third-party evaluations of healthcare institutions and provides a database of medical adverse events and near-miss events with the aim of improving the healthcare system in Japan.

于2023年，日本医疗品质安全委员会（JQ）共收到医疗机构报告的约10.48千例与药物相关的医疗接近事故案例。所谓接近事故，是指几乎发生或实际发生但未导致患者受到伤害的错误。当年，向JQ报告的医疗接近事故总数约为28.88千例。该委员会由国家卫生部门指定，负责对医疗保健机构进行多种第三方评估，并建立医疗不良事件及接近事故数据库，旨在提升日本医疗保健体系的质量。
