Abstract Eosinophilic pustular folliculitis (EPF) is a rare, non-infectious dermatosis characterized by pruritic, recurrent episodes of follicular papulopustules. Three forms of eosinophilic pustular folliculitis have been described: classic EPF (Ofuji disease), immunosuppression- or HIV-associated EPF, and infantile EPF. Histopathologically, eosinophils are frequently observed within and around the follicular epithelium. In our four non-infant patients with negative anti-HIV antibody results, involvement was limited exclusively to the scalp and face. Based on these findings, we suggest that this presentation may represent a distinct clinical subtype of eosinophilic pustular folliculitis characterized by isolated scalp and facial involvement, and we present this case series to highlight this entity.
