Intrapulmonary shunting in patients with COPD and pulmonary hypertension

Pulmonary hypertension (PH) is a serious complication of chronic obstructive pulmonary disease (COPD), particularly in its severe form. Patients with severe COPD-PH often exhibit worse hypoxemia (low oxygen level in the blood) compared to those with moderate PH, despite having relatively milder airflow obstruction. Based on previous research, we hypothesize that the hypoxemia might be caused by the creation of vascular communications (shunts) between the pulmonary and bronchial circulation, bypassing the capillaries. The aim of this project is to use synchrotron-based tomography to explore the presence of these shunts in explanted lungs of patients affected by severe COPD-PH, and compare them to patients with milder forms (as well as controls). If shunts can be identified, we will investigate their regulation by combining imaging with subsequent sectioning and methods like immunohistochemistry and in situ hybridization. The results may lead to novel therapies for this severe condition.
