Defining and discussing spiritual health in primary care and the HOPE tool - A survey of social prescribers

Data from a survey of 171 social prescribers about spiritual health and the use of the HOPE tool. This research seeks to get the views of social prescribing link workers, care coordinators, care navigators and their teams regarding discussing spiritual health, and the use of the HOPE tool. The HOPE tool has been chosen as it is felt it is memorable, short, flexible and applicable to a diverse population
