'Reviewable deaths' are defined by the Community Services (Complaints, Reviews and Monitoring) Act 1993 as deaths of children who die as a result of abuse or neglect, or their death occurred in suspicious circumstances; Children who die while in out-of-home care; Children who die while in detention; Persons with a disability who, at the time of their death, were living in or temporarily absent from, residential care provided by a service provider and authorised or funded under the Disability Services Act 1993 or a licensed boarding house. (1)The New South Wales Ombudsman maintains a register of reviewable deaths, and analyses information about causes of death and any patterns or trends arising from the register. The work is undertaken by the Ombudsman's Community Services Division. The agency conducts research and makes recommendations for the purpose of reducing or removing risk factors associated with preventable deaths.The Registry of Births, Deaths and Marriages provides the Ombudsman with information about the registered deaths of children in NSW. Ageing, Disability and Home Care provides the Ombudsman with information about the deaths of children and adults with disabilities in residential care. The State Coroner's office provides the Ombudsman with information about reviewable deaths and the deaths of children that have been notified to the Coroner. (2)Under the Community Services (Complaints, Reviews and Monitoring) Act 1993, the Ombudsman is obligated to prepare several reports for the House of Parliament: a report of its operations in an annual report, during the previous financial year 1 July to 30 June; a report consisting of data collected and analysed in relation to child deaths registered during the previous year; any other reports tabling the research undertaken. (3)The first report was legislated to be prepared in 2003. (4)Indexes and registers are held by the Ombudsman.Endnotes1.
