VPRS 14967 Index to Sperm Donor Records, Alpha-Numeric System

When the insemination program commenced at Prince Henry's, men who donated sperm prior to 1998 were guaranteed anonymity if they wanted it. Their files were maintained in alpha-numerical order.In the patient records (female) the relevant donor was recorded as alpha-numeric code. (See VPRS 14965 Infertility Treatment Patient Records.)This index can be used to establish the names of men whose background details are described in VPRS 15343 Sperm Donor Files, Non-Identifying Information where they are identified only by an alpha-numeric code.The names of donors in VPRS 14966 Sperm Donor Files, Alphabetical Order by Surname also appear in this index (and so have an alphanumeric code).Note re Date Range:At time of transfer to PROV in 2006 it was understood that this particular index was created by 1990, however it records the codes of donors from 1976 to 1997.
