This excel sheet contains data from an intervention that took place in Geneva, Switzerland between September 1st 2017 and February 28th 2019.
Background: Shared decision making (SDM) processes gathering patients’ and relevant professional’ expertise is needed, especially when patients have complex needs. In 2016, in Geneva, Switzerland, an innovative pilot intervention started implementing transitional SDM interprofessional and interinstitutional processes (IIPs) for patients navigating between a short-stay medical in-patient setting and out-patient/homecare. Project activities included change management actions and the introduction of a nurse coordinator facilitating IIPs.
Aim: The aim of this study was to assess the coverage and fidelity of this pilot intervention. It was approved by the Geneva ethics committee for research (Req-2018-00801).
Methods: We used a post-only evaluation study design. We collected data i) about the characteristics of the populations of patients (no exclusion criteria) and of professionals (short-stay medical unit’s staff, primary care physicians, homecare nurses), ii) about the fidelity and iii) about the coverage of the intervention, using measures such as the complexity of patients’ care needs, the of IIPs (IIPs-multilateral, IIPs-meeting) and the types of actors involved.
