Objective: Acute kidney injury (AKI) is an important feature of thrombotic microangiopathy (TMA). This observational, retrospective, patient cohort study aimed to describe and analyse the characteristics, prevalence, and prognosis of patients with TMA-AKI.
Methods: A retrospective study was conducted in Emergency Department of Peking University People’s Hospital, between January 2017 to December 2021. The patients were assigned to either the AKI group or the non-AKI group.
Results: In total, 27 and 51 patients were placed in AKI and non-AKI groups, respectively. The morbidity rate of AKI was 34.62%. In the AKI group, mortality was 40.74%, while it was 19.61% for the non-AKI group (P = 0.045). AKI was associated with older age (P = 0.033) and an increased infection rate (P < 0.001). Compared with the non-AKI group, the AKI group had increased rates of haematuria (P < 0.001), and proteinuria (P < 0.001). Patients of the AKI group received continuous renal replacement therapy (P < 0.001); however, they received fewer glucocorticoids (P = 0.045). A risk factor for AKI (P = 0.037) was identified as age (P < 0.001) via a logistic regression analysis. In addition, increased total bilirubin level (P = 0.011) and age (P = 0.022) were significantly correlated with an increased risk of in-hospital mortality. Kaplan-Meier survival analysis revealed that for patients of the AKI group, prognosis was poor (P = 0.045).
Conclusion: This study indicates that AKI is commonly observed in those with TMA and increases mortality risk.
