A new surgical approach for placenta previa with severe placenta accreta spectrum

Objective To explore the clinical effect of the Via Posterior Reverse Hysterectomy in the treatment of placenta previa with severe placenta accreta spectrum (PAS).Methods Between January 2022 and July 2024, the Via Posterior Reverse Hysterectomy procedure was performed on 8 patients diagnosed placenta previa with severe PAS at the Third Affiliated Hospital of Guangzhou Medical University. The blood loss, vital signs, transfusion, postoperative recovery etc of the 8 patients were were recorded to explore the clinical effect of this new surgical method.Results The intraoperative diagnosis of 8 cases was placenta previa with severe placenta accreta, which was consistent with the preoperative diagnosis. The vital signs of the 8 patients were stable during the operation, and the intraoperative blood loss was between 600-1500ml, among which 3 patients had more than 1000ml blood loss. 4 patients had transfusion during the operation. The operation duration was 83-114 minutes, and no infection occurred after the operation. All patients were discharged within 8 days after operation.Conclusion: The Via Posterior Reverse Hysterectomy is a good method for hysterectomy of placenta previa with severe PAS, which can effectively control bleeding during the operation.
