ANTIDOTE is an observational study which has two arms. Arm 1 is a longitudinal cohort study of patients with Gram negative bloodstream infection admitted to Queen Elizabeth Central Hospital, Blantyre, Malawi, which aims to understand the mortality, morbidity and economic outcomes for patients who have bloodstream infection with locally untreatable, ceftriaxone resistant ESBL producing enterobacteriaceae (ESBL-E). Arm 2 is a longitudinal cohort of randomly selected households in Blantyre, Malawi, which aims to investigate drivers of transmission and acquisition of ESBL E.coli carriage, using household human faecal sampling with selective ESBL-E culture and whole genome sequencing of isolates. Invasive and carriage isolates from patients, and carriage isolates from community members will undergo whole genome sequencing, allowing for comparison of hospital and community strains, placed in local and global context.
