All predictors were measured at the initial CRP evaluation visit.‡BMI is centered at the overall sample mean (26.7).§There is no observation with history of cancer and repeated CRP elevation.||Male*BMI denotes the interaction between male gender and BMI; male*BMI-squared is the interaction between male gender and the square of BMI.
