The Klebsiella pneumoniae IR27 was isolated from the midstream urine of a 42 year-old male patient visiting the Urology department of Sri Sathya Sai Institute of Higher Medical Sciences, Prasanthigram, India, in April 2016. The patient had consistent left loin pain for over four months. Clinical history included a prior event of spinal cord injury and a neurogenic bladder. Further, the patient also developed fever with chills, gross haematuria and acute flank pain. Due to the presence of kidney stones, ureteroscopic lithotripsy was performed. The patient was diagnosed with narrowing of proximal bulbar urethra and left mid-ureteric calculus. Urine sample was found positive for Klebsiella pneumoniae infection. Vitek-2.0 based AST report suggested that the strain was extensively drug resistant (XDR) and was susceptible only to colistin.
