2016 Community Health Worker Supervisor Quarterly Evaluation Forms, Alabama and Louisiana

The evaluation forms are designed to gauge the effectiveness of the community health workers (CHWs) according to their supervisors. Community health workers’ supervisors completed and submitted forms quarterly to the University of South Alabama’s Coastal Resource and Resiliency Center (CRRC) beginning July 2016 through January 2017. CRRC gathered, input, analyzed, and reported data provided by the CHWs to evaluate the utility of these CHWs in building community resilience.
