Abstract Objective: Evaluation of serum S100β protein as a biochemical marker in identification, progression and outcome evaluation in patients with hypertensive intra cerebral hematoma (ICH). Method: Patients with age more than 18 years with ICH, were included in the study. A total of 47 patients were enrolled. 1st blood sample was taken at the time of patient’s presentation in emergency after confirmation of ICH. Consecutive 2 blood samples were taken at the time of imaging. Outcome was assessed with mRs score. Result: A significant co relation was noted on comparing S100β with volume of hematoma, outcome of patient. Large volume of ICH hematoma showed high value of S100β. In patients with high value of S100β, underwent surgical intervention showed significant decrease in level of S100β (p=0.001). Patients with high value of S100β had poor outcome. Patients who underwent early surgical treatment showed better response.   Conclusion:  S100β showed significant co relation between volume of hematoma, outcome of patients. A sudden rise in the S100β value suggests it to be due to intracerebral bleed.This study can be considered as a pilot study to determine whether S100β could serve as a potential biomarker for diagnosis of ICH, determine progression and functional outcome after ICH. It can also provide information whether patient need to be transfer to higher medical care centre. It can also provide and aprox. value whether patient might need surgical intervention.
