Age is not enough: clinical and therapeutic predictors of survival in elderly patients with de novo glioblastoma

Background: As life expectancy increases, the number of elderly patients diagnosed with glioblastoma is steadily doing so. This study aims to analyse survival outcomes in patients over 65 years old and to identify factors that predict survival to guide management decisions in this population. Methods: A retrospective single-center cohort study included all patients aged ³65 years with de novo histological diagnosis of IDH-wildtype glioblastoma (according to the 2021 WHO classification) treated in our hospital between 2017 and 2024. Patients with incomplete information or loss to follow-up were excluded. Kaplan-Meier curves were used for time-to-death analysis. Predictive factors were assessed by Cox regression analyses.
