A case of fatal nosocomial infection due to extreme-drug resistant NDM-5-producing Klebsiella pneumoniae strain is reported. Bioinformatics studies identified HA30 and HA31 as the same strain colonizing the inpatient at the admission to our institution. HA31 is the first isolate of blaNDM-5, rmtB and blaCTX-M-15 in a colistin-resistant Klebsiella pneumoniae ST11 described in Latin America.
