Medicaid Pharmaceutical Policy Document Database, 2005–2020

This database provides a hand-collected archive of state Medicaid pharmaceutical policy documents from 2005–2020, compiled to establish accurate timelines of pediatric antipsychotic prior authorization policies. This collection includes primary source materials obtained from state Medicaid program records, including prescription drug lists, Drug Utilization Review Board and Pharmacy &amp; Therapeutics Committee meeting minutes, provider manuals, policy memoranda, and related administrative records. <br><br>Although the database was developed to identify the implementation dates of antipsychotic prior authorization policies for children, the underlying documents provide a broad view of state Medicaid drug coverage and utilization management, including preferred drug lists, step therapy requirements, and pharmacy benefit carve-outs. As such, the collection offers a detailed record of how states administered and revised their Medicaid pharmaceutical programs over two decades. Many of these documents were obtained through Freedom of Information Act (FOIA) and Public Record Act requests or recovered from the Internet Archive’s Wayback Machine.
