A Clinical Protocol for Integrating Wenyang Tongluo Detoxification Formula with Immunotherapy and Targeted Therapy in Advanced Gastric Cancer Patients

Advanced gastric cancer remains a major clinical challenge with limited survival outcomes and high treatment-related toxicity. Standard regimens including immunotherapy, targeted therapy, and chemotherapy often improve disease control but are frequently associated with significant adverse effects. To address this limitation, we present a reproducible protocol combining the Wenyang Tongluo Detoxification Formula, a traditional Chinese herbal preparation, with Sindilizumab, Apatinib, and Tegafur-Gimeracil-Oteracil (Tegafur).Sixty patients with advanced gastric cancer characterized by Yang deficiency and toxic stagnation were enrolled and randomly assigned to control or observation groups. The control group received standard therapy consisting of Sindilizumab, Apatinib, and Tegafur, while the observation group received the same regimen combined with the herbal formula. The protocol describes step-by-step procedures for patient selection, herbal decoction preparation, drug administration, and clinical monitoring. Key outcome measures include Traditional Chinese Medicine (TCM) symptom scores, Karnofsky Performance Status (KPS), immune function indices, tumor markers, adverse event profiles, and survival endpoints.Representative results show that patients in the observation group demonstrated greater improvements in TCM symptom relief, functional status, and immune function compared with controls. Moreover, the combined regimen reduced the incidence of leukopenia, anemia, and diarrhea while prolonging median progression-free survival and overall survival. These findings suggest that integrating Wenyang Tongluo Detoxification Formula with standard therapy enhances therapeutic efficacy, mitigates toxicity, and offers a feasible treatment approach for advanced gastric cancer.This protocol provides a reproducible framework for clinicians and researchers seeking to evaluate integrative strategies in oncology and demonstrates the practical value of combining traditional and modern therapeutic approaches in improving patient outcomes.
