Steatotic livers tolerate ischemia-reperfusion injury (IRI) poorly. The purpose of this study was to explore the mechanism of IRI in liver transplantation with severe steatotic donor liver and the effect of heme oxygenase-1 (HO-1) modified bone marrow mesenchymal stem cells (BMMSCs) derived exosomes (HM-exo) on IRI in steatotic grafts.The model of rat liver transplantation with severe steatotic donor liver were established. The culture medium of HO-1 modified BMMSCs were collected and the exosomes were separated by differential centrifugation. After the transplantation was completed and blood flow was opened, PBS and exosomes were injected into the liver through portal vein respectively. Liver tissues were taken 3 days after surgery and transcriptome sequencing was performed.
