Questionnaire for AI Attitudes, AI Anxiety, and Technology Acceptance among Healthcare Decision-Makers in Jordan

This file contains the questionnaire used in the study titled “The Role of Technology Acceptance, AI Anxiety, and Demographic Factors in Jordanian Healthcare Decision Makers' Attitudes Toward Artificial Intelligence.” The questionnaire was designed to assess attitudes toward artificial intelligence, technology acceptance, AI anxiety, and demographic characteristics among healthcare decision-makers in Jordan. The instrument includes demographic questions, the General Attitudes toward Artificial Intelligence Scale, Technology Acceptance Model items, and AI anxiety-related items. The questionnaire was administered in English and Arabic as part of a cross-sectional study.
