The questionnaire of the factors affecting completeness of anesthetic record

Anesthesia Information Management Systems (AIMS) are critical for ensuring comprehensive recording of anesthesia data. The anesthesia care team was familiar with the Medical Record Audit Guideline criteria outlined by Thailand's National Health Security Office (NHSO). Nevertheless, having adequate knowledge alone did not consistently ensure the thoroughness of the anesthetic records. The organization must foster a positive work culture that encourages all staff members to approach their tasks diligently. Moreover, individual factors, particularly among senior anesthetists with extensive experience, can contribute to incomplete documentation of anesthetic records, often due to the demands of urgent situations. As a solution, prioritizing teamwork will facilitate a more systematic approach, minimize errors, and enhance the overall integrity of medical records.
