VPRS 18018 Psychologist's Case History Files

This series consists of case files created by the Psychologist at Winlaton Youth Training Centre and Nunawading Youth Residential Centre (VA 5093).To assist trainees with particular psychological problems, a visiting psychologist periodically attended the centre. A file was created for each trainee who was treated by the psychologist.Each case file includes the following documents:- Copies of the Review Action Sheets presented by Social Workers at trainee progress review meetings- Copies of the Classification Action Sheets which provide a summary assessment of the trainee- Request for Psychological/Psychiatric Assessment Form giving a detailed appraisal of the trainee and why it was felt that they needed treatment- Handwritten treatment and progress notes.Some of the information contained in these files may also be duplicated on the trainee's file in VPRS 17998 Trainee Information Files. This series should be consulted if a file for a trainee cannot be located in VPRS 17998.
