**Objective:** To determine the effect of anti-CD20 antibody targeting the B-cell surface marker CD20 on the occurrence and progression of endometrial cancer (EC), so as to provide references for future clinical treatment.
**Methods:** We conducted two-sample Mendelian randomization (MR) analysis to analyze the causal relationship between EC and immune cell markers (specifically MS4A1), and performed drug target MR analysis to detect the causal association between anti-CD20 antibody and EC risk. The risk of follicular lymphoma was used as a positive control, while EC was used as the outcome.
**Results:** EC exerts effects on 28 immunophenotypes, with a predominant impact on CD20. The expression of CD20 on immunoglobulin D+ CD38- B cells, memory B cells and unswitched memory B cells is elevated after the onset of EC. In addition, we identified an immunophenotype that poses a hazardous effect on EC: CD3/TD CD4+ (mature subset of T cells). Meanwhile, CD20 can act as both a risk factor and a protective factor for EC (p>0.05). Anti-CD20 antibody significantly reduces the risk of follicular lymphoma in two combined GWAS datasets. Moreover, anti-CD20 antibody exerts a protective effect on EC in the two combined GWAS datasets (p>0.05).
**Conclusions:** This study confirms the close association between CD20 and EC. Anti-CD20 antibody may exert a protective effect on EC and reduce the risk of EC onset, providing references for future clinical diagnosis and treatment. However, further clinical validation of our findings is warranted.
