This package contains the informed consent forms used in the study “Utility and Reliability of a Pediatric Preanesthesia Questionnaire: A Monocentric Prospective Study in a Tunisian Public Hospital.”It includes the Arabic version, which was provided to parents and legal guardians of participating children.
