Advancing Precision Diagnostics: Proposing Histo-Integrative Medicine (HIM) to Resolve Complex Idiopathic Disease

Abstract A significant cohort of patients remains trapped in protracted “diagnostic odysseys,” afflicted by complex, debilitating conditions that elude etiological definition within conventional medical frameworks. This failure of diagnostics results in prolonged suffering, psychological distress, and the administration of ineffective, symptom-based treatments. Current diagnostic specialties, particularly traditional histopathology, are optimized for the confirmation of known disease entities but are fundamentally ill-equipped for the *de novo* investigation of multifaceted, idiopathic conditions. This perspective paper argues for the establishment of a new medical specialization: Histo-Integrative Medicine (HIM). We define HIM as a hyper-specialized diagnostic discipline that moves beyond mere confirmation to systematic, first-principles exploration. Its core methodology is the *comparative, multi-modal analysis of pathological versus proximate healthy tissue* from the same patient. By algorithmically integrating inquiries into micro-neurological, metabolic, microvascular, and immunological functions at cellular and spatial resolutions, HIM aims to deconstruct pathophysiology and pinpoint the root cause of enigmatic diseases. Powered by the convergence of spatial omics, computational pathology, and multiplexed imaging, HIM represents a necessary paradigm shift to resolve modern medicine's most challenging diagnostic failures and catalyze the development of truly personalized therapies.
