Data set.<br>Ethical clearance was obtained from the Regional Committee for Medical and Health Research Ethics, Western-Norway (REK Vest), and the National Bioethics Committee of Pakistan. All study participants signed informed written consent. For participants &lt; 15 years of age, the written consent was signed by the parent/guardian.<br>
