ABSTRACT Background: Viral load (VL) suppression in individuals with human immunodeficiency virus (HIV) is influenced by genetic, immunological, and behavioral factors. This study evaluated the relationship between CCR5-Δ32 gene polymorphism, langerin levels, interleukin-12 (IL-12) levels, sexual exposure, and the success of VL suppression in a scoring system. Methods: This cross-sectional study involved 90 individuals with HIV undergoing antiretroviral therapy (ART) in Makassar, Indonesia. Genetic data were obtained through PCR and CCR5 gene sequencing. Langerin and IL-12 levels were measured using the ELISA method. Sexual behavior information was collected through a structured questionnaire. Statistical analysis used chi-square tests and logistic regression analysis. Results: Higher levels of VL suppression were found in females (OR=16; p<0.001), CD4+ >200 cells/µL (OR=6.8; p<0.001), high langerin levels (OR=8; p<0.001), high IL-12 levels (OR=24.7; p<0.001), receptive anal sex (OR=10; p<0.001), insertive anal sex (OR=3.3; p<0.001), receptive vaginal sex-anal receptive sex (OR=1.6; p<0.001), vaginal sex (OR=4; p<0.001), having 1 sexual partner (OR=3.2; p<0.008), and sexual frequency less than twice a week (OR=4; p<0.002). A scoring system was developed through the interaction of Langerin levels, IL-12 levels, and the risk of exposure to receptive anal, insertive anal, and vaginal-anal receptive sex, which was created in the “SUKA scoring” system can be used as a predictor for viral load suppression in PLHIV with a score range of 0-5 and a probability of 7.8%-71.6%,with ROC results from the variable model for predicting probability showing an Area Under Curve (AUC) value of 71.8% with significance p=0.000. Conclusion: HIV viral load suppression is determined by the complex interaction between Langerin levels, IL-12 levels, and the risk of receptive anal, insertive anal, and receptive vaginal-anal sexual exposure created in the “SUKA scoring” and can be used as a predictor of viral load suppression in PLHIV.
