Data collection instruments — Capillary blood glucose testing during early childhood care: a pilot study on operational feasibility and challenges in primary health care

This record contains the data collection instruments used in the pilot study "Capillary blood glucose testing during early childhood care: a pilot study on operational feasibility and challenges in primary health care," conducted in primary health care units in a medium-sized municipality in Goiás, Brazil. The instruments include: Appendix 1: Structured questionnaire applied to caregivers of children aged 1 to 5 years, covering sociodemographic data, family and clinical history, anthropometric measurements, T1DM knowledge assessment (adapted DKN-A Scale), and barriers to T1DM management. Appendix 2: Semi-structured questionnaire applied to 15 health professionals (community health agents and nurses) working in the study units, covering early diagnosis challenges, resource adequacy, family barriers, and communication with caregivers. Both instruments are provided in the original Portuguese version and in English translation for transparency and accessibility. The study was approved by the Research Ethics Committee of the Universidade Federal de Goiás (Protocol No. 6.746.450; CAAE: 76785023.9.0000.5083).
