This study examines the effectiveness of the Living Well in the Community program, a health promotion initiative for people with disabilities. The program aims to improve self-management skills and reduce secondary conditions through a series of structured sessions. Data collection spanned from May 2020 to June 2023, capturing the impact of the COVID-19 pandemic on participants' health and community engagement.<br><br>Anonymized data files are provided in both Excel and SPSS formats. The Excel files describe the cleaning process but lack survey response options, while the SPSS files includes comprehensive data labels.<br><br>Please note that the paper has not been published yet, and all content in this data file consists of the collected data without an accompanying published paper.
