Navigating Leadership Styles in Healthcare: A Scoping Review

Background: Leadership style plays a crucial role in shaping staff performance, morale, and the overall efficiency of healthcare organizations. This scoping review aimed to identify the leadership styles most effective in healthcare settings, synthesize their reported influence on staff motivation, well-being, and organizational outcomes, explore the theoretical frameworks underpinning these styles, and highlight knowledge gaps to guide future research and leadership development initiatives.
Methods: The review was conducted between 1 January 2024 and 30 September 2024 following the PRISMA-ScR guidelines. A systematic search of PubMed and Google Scholar yielded 4,538 records published between 2013 and 2023. After applying eligibility criteria and exclusions, 20 studies were included and analyzed.
Results: Findings indicate that democratic, transformational, and transactional leadership styles were most frequently examined and had the strongest positive impact on healthcare staff. Transformational and democratic leadership enhanced motivation, engagement, and job satisfaction, while transactional leadership contributed to structure and accountability. No single style proved universally ideal.
Conclusions: A blended leadership model integrating elements of democratic, transformational, and transactional leadership styles emerged as most effective. Such an adaptive approach fosters teamwork, resilience, and innovation, ultimately advancing healthcare quality and organizational performance.
