The background for "Keeping Patients out of Hospital by Improving Patient Trajectories in Primary Care - Methodological Development and Effect, 2014" was that data gathered in different registers and journal systems create opportunities for studying patients trajectories. If these data are connected and can be linked to different measures of effect and quality, it would be possible to identify which trajectories that are the most advantageous both from a patient and a system point of view. The purpose of the project was therefore to 1) develop tools to gather and 2) create patient trajectories between different types of services, 3) identify variables that reflect quality from a patient perspective and 4) examine the connection between quality experienced by the patient and different patient trajectories. A register-based longitudinal cohort study with data regarding contact made with somatic health services over a two-year period for 170 000 persons, where a random selection of these consisting of 12 500 with chronic diagnosis were invited to answer question regarding health, function and experienced quality in health services four months after.
