Hepatitis B virus infection represents a colossal global health challenge, primarily due to its insidious, asymptomatic progression in a vast majority of chronically infected individuals. This research aims to elucidate the immunological and virological mechanisms underlying the asymptomatic course of chronic Hepatitis B and to systematically identify high-risk groups within the Andijan region. Utilizing a cross-sectional epidemiological and clinical study design, asymptomatic individuals presenting at regional screening centers and polyclinics were evaluated using serological (HBsAg, HBeAg) and molecular (HBV DNA PCR) diagnostics. The integrated literature review explores the phases of chronic infection, particularly the immune tolerance and inactive carrier phases, where robust viral replication or viral integration occurs in the absence of hepatic inflammation and clinical symptoms. Empirical results reveal that a significant proportion of asymptomatic carriers possess high viral loads despite normal alanine aminotransferase levels, creating a false sense of security. The study identified horizontal transmission within household contacts, individuals undergoing frequent dental or surgical interventions, and hemodialysis patients as the most prominent high-risk groups in the region. The discussion emphasizes that asymptomatic Hepatitis B is not a benign state; continuous, silent viral integration inevitably drives the pathogenesis of liver cirrhosis and hepatocellular carcinoma. Ultimately, the study concludes that relying on symptom-based diagnosis is epidemiologically flawed. Universal screening of identified high-risk populations, coupled with advanced molecular monitoring, is absolutely mandatory to prevent end-stage liver disease and disrupt the silent chain of transmission.
