We report the case of a 51-year-old man with septic shock due to a severe abdominal infection. The patient underwent a peritoneal exploration, ileal repair, and abdominal drainage. After six days of norepinephrine treatment, his distal limbs ultimately developed symmetrical peripheral gangrene.From the viewpoint of inflammatory biomarkers, procalcitonin values decreased gradually, while white blood cell counts increased accordingly, which is not clinically consistent with synchronous changes in most bacterial infections.To examine the role of procalcitonin, we performed a stepwise regression of serum procalcitonin levels and platelet counts and found a highly positive association (R2=0.804; P=0.039), indicating that decreased serum procalcitonin could help explain the decreased platelet count levels.
