Contains three documents: (1) PNDI Encounter Scoring Worksheet — an eight-section structured form for scoring five clinical domains (Narrative Completeness, Meaning Substitution, Salience, Context Stripping, Bias-Driven Distortion) at each clinical encounter, assigning interpretation bands using published cut-points, documenting domain evidence, flagging equity concerns, and tracking longitudinal trends; (2) PNDI Quick-Start Guide — a clinical reference card with domain scoring anchors, five-step implementation guide, interpretation bands, domain-to-bias mapping, and five worked vignettes; (3) PNDI OSCE Teaching Version — a medical education scoring rubric with the Mr. Okafor standardised patient vignette, examiner scoring guide for all five domains, and band-specific feedback prompts.
